dc.description.abstract The study objectives aimed to explore the development of process of continuity care in schizophrenic patient by interdisciplinary team and caregiver network in community of Bandongkating Health Promoting Hospital, Ban Dan District, Buriram Province. Population who participated in this research were schizophrenic patients at 15 years old or above who were registered for treatment at Ban Dan Hospital and Buriram Hospital including living in responsibility area of Bandongkating Health Promoting Hospital for 25 persons, recruited by purposive sampling. Duration period study from October 2019 until September 2020. Mixed method research methodology was taken by Quantitative and Qualitative research integration. Instruments was 4 questionnaires; Bandongkating Health Promoting Hospital Home Visit Record, GAF scale of Department of Mental Health (2014), WHOQOL-BREF-THAI of Suwat Mahatnirankul et al. (2014) and Chronic Schizophrenic Patient in Community Follow Up: 10 Dimensions Form of Chitchanok Opartwattana (2020). Collecting data by 4 questionnaires together with focus group and in-depth interview among key persons; patient, their family, community leader, local organization officer, village health volunteer, interdisciplinary team and caregiver network in community and local leader. Length of collecting data last 3 months from June to August of 2020. Analyzing quantitative data by frequency, percentage, mean and standard deviation for qualitative date was analyzed by content analysis. Results found that schizophrenic patients were male 20 persons (80.0%), 30-45 years old 13 persons (52.0%), single 16 persons (64.0%), duration of illness more than 2 years 22 persons (88.0%), living in community more than 5 years 24 persons (96.0%), not drinker 19 persons (76.0%), cigarette use 14 persons (56.0%), normal condition 23 persons (92.0%), daily routine efficacy 22 persons (88.0%), Housework efficacy 16 persons (64.0%), earn living 15 persons (60.0%), having care obstacle 18 persons (72.0%) such as lack of caregiver, drug abuse, inappropriate health belief of cure, GAF Scale at maximum 12 persons (48.0%), moderate quality of life 18 persons (72.0%). After the development of process of continuity care in schizophrenic patient 1 month later, follow up the participants was proceed and found that they earn living ( = 1.08, SD = 0.28), moderate environment ( = 2.16, SD = 0.75). Some participant reported satisfied to this study caused it’s valuable and benefit. Recommendation were combination care system with network, caregiver and community, information technology development and further study in field of district region.
